Goal discussion:
This week, you and the student are going to discuss goals for the
year, and strategies to attain those goals. Be careful
not to impose your own goals for the student, but guide the student
to pick positive and realistic goals. Fill-out the goal
setting plan below with the student.You may want to review the
discussions from previous sessions as you complete the goal setting
sheet.
Student Worksheet 2: Goal Setting Plan: (Based on Heacox, 1991)

want to improve? (This is your long term goal. It may take you several
weeks, months, or even a whole school year to improve this goal.)
